Are you looking for a better Work-Life Balance? The part-time Registered Nurse Case Manager works with our Director of Nursing and Branch Manager out of our well-established BrightStar of New Brunswick office and is responsible for providing care management services in a home care environment.

The RN Case Manager is responsible for overseeing the care provided to our clients as directed by BrightStar Care and our Joint Commission accreditation quality and safety guidelines.

Responsibilities:
  • Work with the president of the company to define overall patient care policies and framework
  • Manage a team of nurses and assist in hiring decisions
  • Provide care as defined within the scope of the New Jersey nurse practice act
  • Make initial evaluation visits and regularly reevaluate clients’ nursing needs
  • Prepare clinical and progress notes and summaries of care
  • Assist in the development of organization goals. Develop, recommend, and administer agency policies and procedures
  • Supervise, educate, and coach other nursing personnel and caregiving staff
  • Plan and implement in-service and training programs
  • Oversee the maintenance of client clinical records, statistics, reports, and records for purposes of evaluation and reporting of organization activities
  • Stay informed about changes in the field of nursing and home health care. Share information  with appropriate organization personnel
Requirements:
  • Current unrestricted RN license in the state of Illinois
  • Certified CPR and negative TB skin test or chest x-ray
  • Minimum of one (1) year of documented experience required. Two (2) or more years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred
  • Demonstrated ability to supervise and direct staff
  • Licensed driver with valid automobile insurance
  • Excellent Computer Skills, 100% Digital Charting
  • Infusion Experience is a Plus!
  • Excellent customer service skills
  • Treat clients, staff and the public with courtesy, respect and presents a positive public image
  • Case management skills a plus
  • Ability to travel including weekends, nights and holidays if necessary
  • Understands regulatory compliance such as OSHA and HIPAA
  • Experience with, or desire to, participate in marketing and sales a plus
  • Flexibility and good time management skills A MUST
  • Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull. Varied assistance is required depending on the mobility of the client.  Must be able to lift up to 50 pounds
